What to write in an email to an old Prof?

I want to write to my former professor a short note to ask her if we can schedule an appointment to talk about some teaching degrees i’ve been looking into and want to apply to. I need to remind her who I am & what class I had with her because it’s been 3 years since we last talked and since i last had the class with her. How can I say all of this in a short email?

Dear professor X

i hope you don’t mind me contacting you after all this time. I took your xxx class in the academic year beginning xxxx. I wondered if it would be possible to arrange a time to talk to you about some teaching degrees that i’ve been looking at.
